Integrating care across disciplines and organisations around the needs of the person with diabetes has been proposed as an approach that could improve care while reducing cost- but has it and can it? Integrated Diabetes Care- A Multidisciplinary Approach collates evidence of worldwide approaches to both horizontal integration (across disciplines) and vertical integration (across organizations) in diabetes care and describe what was done, what worked and what appeared to be the barriers to achieving the goals of the programmes. Evidence is sought from groups who have developed different approaches to integrating diabetes care in different health systems (eg insurance vs tax payer funded, single vs multiple organization, published vs unpublished). A final chapter brings the evidence together for a final discussion about what seems to work and what does not.

BackgroundDiabetes is a complex disease with some complexities due to diabetes itself and some by individual response to living with illness. Some of these patients with difficult to manage diabetes are seen in specialist centres however they soon become disengaged and stop attending specialist services. In Birmingham UK integrated diabetes care is in place for last 6 years, part of this service involves having case based discussion as virtual clinic and other half as face to face contact. A number of patients were identified with persistent poor glycaemic control with previous contacts with community or hospital specialist services. This audit is to look at the impact of reengaging these patients at general practice level.AimsTo see the change in HBA1c, one year after initial contact with such patients.MethodAll patients seen jointly with general practioner (GP), diabetes consultant, nurse specialist (DSN) and practice nurse (PN) in 3 GP practices were identified.criteria for selection.1. Had HBA1c > 75 mmol/mol for more than 16 months.2. Previous contacts with specialist teams.3. HBA1c was available 1 year after original contact.Total of 189 patients were seen face to over period of 3 years. 69 patients full filled above criteriaInitial consultation involved 30 minutes consultation in which main emphasis was listening to person with diabetes story. Options were discussed rather than forced. Specialist services provided reassurance of continuity and support by same health care professionals. Subsequent follow up were done either with in general practice by GP/PN or DSN. Some patients were seen again by consultant either in general practice or community specialist centre.Resultstotal 69 patients 38 males 31 female 3 patients had type 1 rest type 2 diabetes, Mean age 53. Base line mean HBA1c 97 mmol/mol which improved to 74 mmol/mol10 patients no improvement rest of 59 patients HBA1c improved from 108 mmol/mol to 71 mmol/mol38 patients 55% were able to achieve their individualised target.DiscussionIntegrated diabetes care provides ideal opportunity to address unmet diabetes need at personal and population level. Disengaged complex patients eventually end up a lost tribe. We undertook this method of engagement as it provides fresh start, rather than emphasizing on medical individualisation, individualisation at personal level was explored. Most literature on disengagement exits for patients with mental health issues or infectious disease. One study (1) in type 1 diabetes population indicated that young, male, person with anxiety/depression and multiple deprivation likely to become disengaged no intervention methods to improve glycaemic control have been studied in detail. In our cohort 46% either had a diagnosis of mental health issues.UK National Health Service is universal health care and has to manage complications irrespective of how they appear. This cohort provide a significant pool from where patients end up having long term complications. The intervention gives an opportunity to not only improve health of individuals but also to reduce economic burden on health care system, however this needs to be confirmed in larger population comparison with control, and over longer period.References1.Adults with type 1 diabetes: what factors areassociated with disengagement from health services? BACKGROUND. Gyeonggi province, the largest province in Korea, was developing integrated interventions for elderly patients with diabetes at five HDRM(Hypertension/Diabetes Registration and Management) centers and 10 community public health centers in Gyeonggi province since 2017.PURPOSE. The purpose of this pilot study is to assess the feasibility of a community-based integrated diabetes self-management program(IDSMP) for elderly patients with diabetes comparing treatment participants to a randomized usual-care control group at 12-weeks. This IDSMP was consisted of two lectures on principles of diabetes management, self-monitoring blood glucose level for 12 weeks and five times face-to-face lifestyle counseling delivered by registered nurses and dieticians at five HDRM centers in Gyeonggi province, Korea.METHODS. A total of 284 elderly patients with type 2 diabetes were randomized to a usual-care control group or 12-weeks integrated diabetes self-management program(IDSMP) for elderly patients at five HDRM centers in 2019. Randomized participants were compared at 12 weeks. HbA1C, blood pressure and BMI were measured at baseline and 12-weeks. All other data about diabetes knowledge, self-efficacy and self-management were collected by standardized questionnaires. Usual-care which local hypertension/diabetes registration and education centers at five HDRM centers carry on for diabetes care included two lectures on principles of diabetes management and self-monitoring blood glucose level for 12 weeks.RESULTS. At baseline, no significant differences in baseline characteristics were observed between the intervention and control groups. HbA1C of patients allocated to the intervention group were higher than controls however the difference was not significant statistically. Within-group comparisons showed weight, BMI, systolic blood pressure, diastolic blood pressure and HbA1C improved at 12-weeks in the intervention group(p

The percentage of the population diagnosed with diabetes doubled between 1994 and 2009 and is continuing to increase which could have a significant impact on NHS resources. Too many diabetic people are developing complications because they are not receiving the straightforward care and support they need. The Department estimates that 24,000 people with diabetes die prematurely each year because their diabetes has not been managed effectively and it's estimated 80% of the costs of diabetes in the NHS are attributable to the treatment and management of serious, yet avoidable, diabetic complications. These costs will continue to increase as the number of diabetic people rises as will premature deaths. The National Service Framework for Diabetes, 2001 set out minimum standards for good diabetes care, including nine basic care processes which check for the early signs of avoidable diabetic complications as well as treatment targets for the management of blood glucose, blood pressure and cholesterol to minimise the risk of diabetic complications developing. Progress in delivering these standards of care and in achieving treatment targets has been poor. There is no strong national leadership, no effective accountability arrangements for commissioners, and no appropriate performance incentives for providers. The Committee saw no evidence that the Department will ensure that these issues are addressed effectively in the new NHS structure. Variation in the level of progress across the NHS also means that there is an unacceptable "postcode lottery" of care
