London has high rates of STI and HIV diagnoses, variation in access and activity across boroughs, an annual spend of over u00a3140m and 1.4m clinic attendances. A partnership of London boroughs u2013 London Sexual Health (LSH) u2013 was created to evaluate and re-invent this public service. The objective was to deliver the best possible service and outcomes while making best use of resources. An engagement project involving 5000 residents helped inform the shape of services. A vision was developed with shared objectives including a single online STI testing service. The aim was to support service users towards self-management and away from face-to-face screening where appropriate, enabling clinics to focus on delivering care to vulnerable and complex patients. Over the past 2 years, councils clustered into 7 u2018sub-regionsu2019 to develop local services within the context of the overall programme aims of LSH. A modified tariff linked to clinical coding was adopted, and integrated sexual health and contraceptive services were commissioned. There is a robust London-wide governance structure, with senior clinical input as an essential part of the process. The new integrated clinical specification predicated on best practice pathways was developed through a clinical advisory group. A common tariff structure supported integration of STI and contraceptive services. Terrestrial service specifications were required to integrate with a pan-London online clinical service offering more choice and increased access for service-users seeking STI screening. The e-service, launched in January 2018, will become the largest online clinical service in the UK. LSH has delivered a partnership approach to improve the sexual health of London residents by the establishment of robust, sustainable clinical and commissioning collaborations across the capital, enhancing the use of digital technology and self-care, and recognising the different ways that people now live their lives and access services.
