The reactions to the German edition of this book have been most contradic tory. While some readers found the book absolutely unnecessary ("Surgery is learned from practice, not from theory"), others were enthusiastic (" Just the sort of book I have always needed "). In my opinion the divergent recep tion of the book is best explained by the comment of my friend, BALDER GLOOR *, who called the book a "grammar of eye surgery". Indeed there are people who can learn a language by practice alone, and there are others who do better by knowing its grammar. In learning by mere practice to be sure, proficiency will be long in coming, because it will be more difficult to recognize, and thus to correct, whatever errors are made, whereas a knowledge of grammar will reveal the basic structures and shorten the learning process. Similarly, surgery can be learned purely from practical experience. Yet the trial-and-error quest for experience is not compatible with the interests of the patient. A knowledge of "surgical grammar", on the other hand, not only shortens the time for learning but also helps us to compare different surgical techniques and to weigh thcir advantages and disadvantages. It is also helpful in developing new surgical methods, for the general rules of course, can always be applied to novel situations where experience, is lacking.

This short book is an account of an approach to retinal detach ment surgery. I hope it will be of use to all those dealing with the detached retina, although it has been written mainly for the training ophthalmologist, who, becoming familiar with the methods of examination, is faced with a bewildering array of methods of treating the detached retina. The object of surgery is to produce the desired result in the least traumatic way possible and this involves correct interpretation of how the detached retina is likely to respond to the operation that is planned. I have therefore concentrated on the aspects of clinical examination that have a direct bearing on the planning of the type of operation to be performed. I have not attempted to deal with all the different surgical techniques currently practised, but have described only those that I believe represent the simplest, safest, and most effec tive methods. I have drawn widely on standard techniques; thus, for examination, Schepens' indirect ophthalmoscopy with scleral depression, Lincoffs use of cryotherapy and his modification of Custodis' non-drainage techniques at operation and Rosen gren's use of intraocular air. Because this book is not intended for the retinal surgical expert I have not dealt in great depth with rare cases, e. g. macular holes, or other conditions that are exceptionally difficult to treat (massive periretinal proliferation).

Over the weekend of 21-23 February 1997, a small group gathered in Tallahassee, Florida, at the invitation of Ted Williams, to discuss "photo stasis and related topics." The majority of participants were former students and colleagues of Ted's, but an occasional outsider such as myself was generously included. The papers presented there are collected in this volume. The theory of photo stasis was first outlined in a landmark paper by John Penn and Ted, published in 1986 in Experimental Eye Research. They provided compelling data showing that, in the albino rat eye, levels of rhodopsin, outer-segment length, rhodopsin regeneration rate, and even, perhaps, rhodopsin packing density all depend on the levels of cyclic illumination (12 hours light, 12 hours dark) in which the animal was reared. So, for example, there is fourfold less rhodopsin in a retinal extract derived from an animal reared at 400 lux than in an extract from the retina of an animal reared at 3 lux. Animals reared at intermediate levels of light show intermediate amounts of rhodopsin that are correlated with illumination level. What these data immediately suggested is that the photoreceptor cell can adjust its photon-catching ability in response to the levels of light in which an animal is reared, and they also provided a compelling rationale for outer-segment turnover, a phenomenon discovered 20 years earlier by Richard Young but whose function has remained obscure.
