On Tuesday 25 July 2000 Air France Flight AFR 4590, a Concorde registered F-BTSC, took off from Paris Charles de Gaulle, to undertake a charter flight to New York with nine crew members and one hundred passengers on board. During takeoff from runway 26 right at Roissy Charles de Gaulle Airport, a tyre was damaged. A major fire broke out. The aircraft was unable to gain height or speed and crashed onto a hotel, killing all 109 people on board and 4 on the ground. The crash would become the end of the Concorde era.

On May 25, 1979, American Airlines Flight 191, a McDonnell-Douglas DC-10-10 aircraft, on its way from Chicago to Los Angeles, crashed just after take-off near Chicago-O'Hare International Airport, Illinois. During the take off the left engine and pylon assembly and about 3 ft of the leading edge of the left wing separated from the aircraft and fell to the runway. Flight 191 crashed killing two hundred and seventy one persons on board and two persons on the ground. The accident remains the deadliest airliner accident to occur on United States soil.

On August 12, 1985, a Japan Airlines B-747 aircraft lost, shortly after take-off, part of its tail and crashed in the mountains northwest of Tokyo. Of the 524 persons on board 520 were killed, 4 survived the accident. The accident was caused by a rupture of the aft pressure bulkhead of the aircraft, and the subsequent ruptures of a part of the fuselage tail, vertical fin and hydraulic flight control systems. The rupture happened as the result of an improper repair after an accident with the aircraft in Osaka, in June 1978.

On 25 January 2010, at 00:41:30 UTC, Ethiopian Airlines flight ET 409, a Boeing 737-800, on its way from Beirut to Addis Abeba, crashed just after take-off from Rafic Hariri International Airport in Beirut, Lebanon, into the Mediterranean Sea about 5 NM South West of Beirut International Airport. All 90 persons on board were killed in the accident. The investigation concluded that the probable causes of the accident were pilot errors due to loss of situational awareness. Ethiopian Airlines refutes this conclusion. Other factors that could have lead to probable causes are the increased workload and stress levels that have most likely led to the captain reaching a situation of loss of situational awareness similar to a subtle incapacitation and the F/O failure to recognize it or to intervene accordingly. Ethiopian Airlines refutes the investigation. According to the airline the final report was biased, lacking evidence, incomplete and did not present the full account of the accident.

On February 24, 1989, United Airlines flight 811, a Boeing 747-122, lost a cargo door as it was climbing between 22,000 and 23,000 feet after taking off from Honolulu, Hawaii, en route to Sydney, Australia with 355 persons aboard. As a result of the incident nine of the passengers were ejected from the airplane and lost at sea. The cargo door was recovered in two pieces from the ocean floor at a depth of 14,200 feet on September 26 and October 1, 1990. The probable cause of this accident was a faulty switch or wiring in the door control system. Contributing to the cause of the accident was a deficiency in the design of the cargo door locking mechanisms. Also contributing to the accident was a lack of timely corrective actions by Boeing and the FAA following a 1987 cargo door opening incident on a Pan Am B-747.

On January 31, 2000, Alaska Airlines, Flight 261, a McDonnell Douglas MD-83, was on its way from Puerto Vallarta, Mexico, to Seattle, Washington, when suddenly the horizontal stabilizer of the plane jammed. While passengers were praying for their life, Captain Thompson and First officer Tansky tried to make an emergency landing in Los Angeles. They did not make it, the plane suddenly crashed into the Pacific Ocean, killing all 93 people aboard. On April 15, 2002, Air China flight 129, a Boeing 767-200ER, operated by Air China, en route from Beijing, China to Busan, Korea, crashed on Mt. Dotdae, near Gimhae Airport, Busan. Of the 166 persons on board, 37 persons survived the crash, while 129 occupants were killed. The Korean Aviation Accident Investigation Board (KAAIB) determined that the probable cause of the crash was pilot error due to poor crew resource management and lost situational awareness during the circling approach of the runway. The Chinese investigation team pointed out that the Korean ATC was not fully licensed and mistakenly directed the airliner to descend to a wrong altitude and that the airport did not inform the crew of the weather conditions at the time. A contributing factor was that the airline made all announcements in Chinese and English, while most passengers were Korean.

About 2046 eastern daylight time on August 16, 1987, Northwest Airlines flight 255, a McDonnell Douglas DC-9-82, a regularly scheduled passenger flight en route to Phoenix, Arizona, crashed shortly after taking off at the Detroit Metropolitan Wayne County Airport, Romulus, Michigan. The airplane collided with obstacles northeast of the runway when the left wing struck a light pole located 2,760 feet beyond the end of the runway. The airplane broke up as it slid across the ground and postimpact fires erupted along the wreckage path. Of the 155 people on board passengers only a 4-year-old child, survived.. On the ground, two persons were killed. The National Transportation Safety Board determines that the probable cause of the accident was the flight-crew’s failure to use the taxi checklist to ensure that the flaps and slats were extended for takeoff. Contributing to the accident was the absence of electrical power to the airplane takeoff warning system.

During the course of the four and a half year investigation of the crash of USAir Flight 427 near Aliquippa, Pennsylvania, killing 132 people, the NTSB discovered that the PCU's dual servo valve could jam as well as deflect the rudder in the opposite direction of the pilots' input, due to thermal shock, caused when cold PCUs are injected with hot hydraulic fluid. On Tuesday 24 March 2015, the Airbus A320-211 registered D-AIPX operated by Germanwings took off from Barcelona, Spain, at 09:00 with destination Düsseldorf, Germany. At 09:41, the aircraft crashed into the mountains northeast of Marseille. The investigation into the causes of the crash revealed that the co-pilot, at a moment when he was alone in the cockpit, had deliberately flown the plane into the mountains killing all 150 persons on board. The investigation revealed also that the co-pilot was under medical treatment for depressions by several health care providers. Neither of those providers informed any aviation authority, nor any other authority about the co-pilot’s mental state. No action could have been taken by the authorities and/or his employer to prevent him from flying on the day of the accident, because they were not informed about the co-pilot’s mental state of mind.

On 28 December 2014 an Indonesia AirAsia Airbus A320-216 aircraft registered as PK-AXC was cruising at 32,000 feet on a flight from Juanda Airport, Surabaya, Indonesia to Changi Airport, Singapore with total occupants of 162 persons. The Pilot in Command (PIC) acted as Pilot Monitoring (PM) and the Second in Command (SIC) acted as Pilot Flying (PF). The Flight Data Recorder (FDR) recorded that many master cautions activated following the failure of the Rudder Travel Limiter which triggered Electronic Centralized Aircraft Monitoring (ECAM) message of AUTO FLT RUD TRV LIM SYS. The crew tried repeatedly to reset the computers but the autopilot and auto-thrust disengaged and the flight control reverted to Alternate Law. The aircraft stalled and crashed. The investigation showed that the loss of electricity and the RTLU failure were caused by a cracked solder joint. All occupants of the plane were killed in the accident.

The Lockheed 1011 registered A6-BSM, operated by Star Jet and chartered by Olympic Airlines, arrived on 4 July 2005 at Terminal 1 at Paris Charles de Gaulle airport. Departure was delayed because the forward hold door could not be closed. A mechanic tried to close the door manually with a hammer and a chuck. Some passengers, worried about the apparent state of the cabin and the noise, asked to disembark, and this led to a mass movement. The airplane took finally off at 16h17. Shortly after departure the crew noticed problems with engine number 3. The captain requested the SEVERE DAMAGE procedure and returned to the airport. The French Bureau d'Enqu tes et d'Analyses pour la s curit de l'aviation civile (BEA) investigated the incident. BEA found out that the aircraft suffered from many problems, such as leaking fuel, malfunctioning safety features and lacking maintenance. The flight crew was not properly licensed, the captain was too old to fly in Europe. The Lockheed Tristar was a flying coffin.

On March 23, 2004, about 1918:34 central standard time, an Era Aviation Sikorsky S-76A helicopter, N579EH, crashed into the Gulf of Mexico about 70 nautical miles south-southeast of Scholes International Airport (GLS), Galveston, Texas. The helicopter was en route to the drilling ship Discoverer Spirit. The captain, copilot, and eight passengers aboard the helicopter were killed, and the helicopter was destroyed by impact forces. The flight was operating under the provisions of 14 Code of Federal Regulations Part 135 on a visual flight rules flight plan. Night visual meteorological conditions prevailed at the time of the accident. The National Transportation Safety Board determines that the probable cause of this accident was the flight crew's failure to identify and arrest the helicopter's descent for undetermined reasons, which resulted in controlled flight into terrain.
